- Customer identification number (if applicable)
- Location/site of installation
- Case number (if applicable)
- The product and version of software you are using
- The operating system you are running (Windows 95,
98, NT etc.)
- A description of the issue including:
- What you were trying to do when the problem
occurred
- Any error message that may have appeared
- Any steps you took to try to resolve the problem
- Additional information required to reproduce
error, e.g. DICOM file
